The social survey - the first essential element for obtaining a disability certificate
The social investigation is the first step in the preparation of the file on the classification of the degree and type of disability of any disabled person, child or adult.
Purpose of the social survey is to establish the status and social context in which the person under assessment lives, being necessary to complete the documentation in order to obtain the certificate of classification in degree and type of disability and is submitted to the General Directorate of Social Assistance and Child Protection (DGASPC) Mures, Targu Mures, 7 Trébely Street.
In the case of persons domiciled or residing in the Municipality of Targu Mures, the social investigation is carried out by the Targu Mures Social Assistance Department, based on a written request, addressed by the adult person applying for disability classification or by his/her legal representative, and in the case of a child, by the parent or the child's legal representative.

The application for a social survey shall be submitted at the Social Welfare Department's Registrar's Office, Gh. Doja, nr. 9, working hours with the public: Monday-Friday: 08:00-12:00, Tuesday and Thursday: 14:00-16:00 or by e-mail at: registratura.das@tirgumures.ro, and shall be accompanied by the following documents:
- Xerox copy of I./C.I./C.N. (for all family members);
- Income statements/pension coupons (for all family members earning income);
- Medical report from the family doctor (which will include: onset of illness, detailed current condition (complications, course of illness, recommendations/proposals) or Summary Sheet (for minors);
- Medical letter from your doctor (detailing the illness);
- Institutional route sheet - from school (for minors);
- Psychological assessment form (for minors).
In the case of minors, the social investigation is also carried out to obtain the certificate of school and professional orientation of the disabled child.
