Law No 448/2006, on the protection and promotion of the rights of persons with disabilities with subsequent amendments and additions.
In order to obtain the social inquiry, the applicant (the person concerned, if applicable, his/her family, legal representative, personal assistant, any person representing him/her) shall submit the application form with the necessary documents to the Social Welfare Department of Targu Mures.
The social inquiry is carried out at the person's/the minor's home by a committee of two inspectors from the department, establishing the status and social context in which the person lives.
The original of the social survey will be handed to the applicant on signature. One copy shall be filed and two copies shall be handed to the applicant, as an annex to the file to be submitted/submitted by the applicant to the Commission for the Assessment of Adults with Disabilities or, in the case of children, to the Complex Child Assessment Service.
Documents required for social assessment for disability classification
- Standard request;
- Xerox copy of B.I./C.I./C.N. (for all family members);
- Income statements/pension coupons (for all family members earning income);
- Medical report from the family doctor (which will include: onset of illness, detailed current condition (complications, course of illness, recommendations/proposals) or Summary Sheet (for minors);
- Medical letter from your doctor (detailing the illness);
- Institutional route sheet - from school (for minors);
- Psychological assessment form (for minors).
