In order to obtain the social survey necessary to complete the documentation for school and professional orientation, the parent/legal representative of the child shall contact the Social Assistance Department of Targu Mures.
In order to carry out the social investigation, a visit to the child's home is carried out by the Directorate's inspectors.
The original of the social survey will be handed to the parent/legal representative of the child, based on his/her signature.
Documents required for the Social Survey in order to obtain the School Guidance Certificate
Typical request;
Copy of the child's birth certificate or identity card;
Copies of parents'/legal representative's identity documents;
Income statements/pension coupons (for all family members earning income);
Copy of the document proving that the person signing the application is the child's legal representative, e.g. court order or decision of the child protection committee;
Xerox copy of Summary Medical Record (prepared and issued by family doctor);
Psychological assessment sheet (assessed and issued by a clinical psychologist);
- Medical report (issued by specialist doctor);
A5 medical certificate;
Psycho-pedagogical sheet (prepared by teaching environment and issued by the educational establishment);
Photocopy of the School and Vocational Guidance Certificate (in the case of expired Certificate).
